Take One Card

One initiative of the information and resource campaign is distribution of a Senior Quick Reference “take one” card. This card has helpful resource numbers seniors may keep by their home phone. The Take One Card is helping the community, particularly older adults and their caregivers, to have better knowledge of how to access health and long-term care information and resources. Over 40,000 cards have already been distributed throughout the area to hospitals, service providers, grocery stores, and libraries.

Here are just a few of the locations where Take One Cards can be found:
  • Any library in Duval, Clay, Nassau, or Baker Counties
  • Baptist Health Campuses
  • Brooks Rehabilitation
  • Shands Hospital
  • Jewish Family and Community Services
  • Mary Singleton Senior Center
  • Catholic Charities
  • AARP Employment Center
  • Social Security offices in Duval and St. Johns Counties
  • Veterans’ Administration Offices in Duval, Clay, Nassau, and St. Johns Counties
  • Winn Dixie stores
  • Baker County Health Department
