Take One Card
One initiative of the information and resource campaign is distribution of a Senior Quick Reference “take one” card. This card has helpful resource numbers seniors may keep by their home phone. The Take One Card is helping the community, particularly older adults and their caregivers, to have better knowledge of how to access health and long-term care information and resources. Over 50,000 cards have already been distributed throughout the area to hospitals, service providers, grocery stores, and libraries.
